Before Your Surgery
Palos Hills Surgery Center has chosen One Medical Passport to enable patients to complete their health history online. This can be done from any computer – your home, work, your surgeon’s office, public library, or that of a family member if they are helping you complete the information. If you are unable to complete the online health history yourself, a nurse from the surgery center can call and assist you.
You will also be able to print out, or receive by mail, your pre-op instructions at that time. Then our Pre-Operative Assessment nurse will access the information you entered and use it to prepare for your surgery.
